How do I refer a case to Dr. Haley?

• Fill out, print and fax or email the Client Referral Form to Dr. Haley.  (The fax # is on the bottom of the form).
• Fax a copy of the diagnostic test results (blood work, etc) to (603) 488-5574.
• Please send a copy of the x-rays with the owner.
